CREDIT CARD AUTHORIZATION AGREEMENT

• I am hereby completely satisfied with the overall work performed by All Week Plumbing And Heating Inc.
• I understand and agree that special order items are not subject to cancellation, modification or return, and that all claims must be made with in 24 hours upon completion of services.
• All Week Plumbing and Heating Inc., agrees to perform the services requested and agreed to by the cardholder.
• The cardholder understands that they are requested to be present to accept these services.
• The cardholder waives any and all right to make a claim if they are not present at the time of service.
• I understand that All Week Plumbing will be charging my Credit Card an additional 4% added to the grand total, as a Credit Card handling fee.
• No claims will be accepted after service completion.
• Any Guarantee or warranty of the parts and services provided by All Week Plumbing and Heating Inc. is limited to that offered by the manufacturer of the merchandise, and will be subject to the terms and conditions of the manufacturer’s warranty.

A copy of facsimile and or email of this Authorization Agreement will be considered as effective and valid as the original.

INFORMATION GIVEN BELOW MUST MATCH THE CREDIT CARD BILLING STATEMENT.
